Thank You For Your Order!

You’re KEY Root-Cause Mini Blood Work Panel order has been received.

The next step is to email [email protected] with your birthdate. We need this to place your order with the lab and send you a lab requisition form. In a day or two you after you receive your lab requisition form, make an appointment with the lab for your blood draw.

Here are next steps:

STEP 1: Find a LabCorp closest to you - CLICK HERE.

STEP 2: Make an appointment as early in the morning as you can.

STEP 3: Take your ID with you to the lab so they can verify your requisition. The doctor named on your requisition form is Blankenship, Will MD. He is the doctor in charge where we order your labs.

STEP 4: Fast the morning of the test. Stop eating 12 hours before your blood draw, but be sure to drink plenty of water the morning of your test so you are hydrated (dehydration is stressful on the body).

STEP 5: We will email you your lab test results once they’re complete.
